Applications and Team Member Forms

To apply to come on mission, complete and mail to us the following:  (Scroll down to download forms)

1.  Application Form

2.  Application Fee:  $150 (additional members of the same family

     are $50 each)

3.  If you are under Age 18, a notarized Parental Consent Form

     OR

     If you are Age 18 or older, a Background Check Authorization Form

When your application is received, we will mail a Pastoral Reference Form to your pastor.  We will send you a confirmation letter when we receive your application and a team assignment letter about one month before mission begins.
